POST: Assistant Care Manager
PURPOSE OF POST: To support the Manager’s role in providing high quality care and support to SweetTree clients and assist in managing the Support Worker team. To assist in ensuring a high level of compliance to SweetTree’s Quality Assurance markers and to play a supporting role in ensuring relevant company KPI’s are met.
RESPONSIBLE TO: Head of Service
PRINCIPAL DUTIES MAY INCLUDE, BUT ARE NOT LIMITED TO, THE FOLLOWING:
✨ Responsible for auditing the enactment, accuracy and day-to-day delivery of support as dictated by the Support Plans, including outcome monitoring and medication provision
✨ Responsible for auditing Daily Support Records and escalating any concerns to the Support Manager
✨ Responsible for delivering client care reports to third parties (e.g. Solicitors, Case Managers, Local Authorities) when required
✨ To assist the Manager in supporting the implementation of ST Connect within the Dementia service
✨ To assist the Dementia Manager in undertaking spot checks and client reviews, as required
✨ To participate in needs assessments, including basic risk assessments, and assist in preparing and updating care and support plans
✨ To assist the Manager with administrative tasks, including maintaining up-to-date documentation and note-taking for investigation meetings
✨ Responsible for maintaining and updating the Care Certificate log sheet for new support workers, ensuring the 12-week target is met
✨ Responsible for support worker competency spot checks and probation reviews
✨ To take a proactive role in developing Support Workers’ learning and development and assisting with performance management
✨ To assist the Manager in interviews and take a leading role in cross-service interviews, as required
✨ To assist the Manager in delivering facilitation sessions for Support Workers
✨ To help ensure coordination of care packages and shift delivery is effective
✨ Responsible for helping answer the New Enquiry Line
✨ To act as backup contingency for emergency client support during uncovered daytime shifts
✨ To participate in the On-call Out of Hours rota as required
✨ To actively answer telephone calls in the SweetTree office
Essential and Desirable Experience, Skills or Attitudes:
✅ Essential – Experience working in a team within a pressurised, fast-changing environment
✅ Desirable – Experience of working as a support worker in the community and understanding the challenges faced
✅ Desirable – A relevant administrative or IT qualification
✅ A proactive, “can-do” attitude that embraces challenges
✅ Strong interpersonal skills with the ability to motivate a remote workforce
✅ Strong dispute resolution skills and the ability to remain calm under pressure
✅ Experience of working positively in a diverse environment
✅ Ability to communicate clearly and tactfully at all levels, both verbally and in writing
