At SweetTree Home Care Services we
recognise that many people with learning disabilities live active, independent
lives however, we also know that for many others, obtaining the right support
can be the difference between reaching ones full potential and living a life of
challenge and isolation.
A learning disability should not
prevent some one continuing to learn or from living life to the full nor does
it define their personalities, likes, dislikes, goals and ambitions. At
SweetTree we value the belief that people with learning disabilities are people
first. We focus on what people can do, with support where needed, rather than
on what people cannot do, and we work to empower them to live as full and
independent lives as possible.
Consistent with our Person
Centred philosophy of care, the aim of the SweetTree team is to support
clients to make their own decisions and then benefit from these choices within
normal community setting. We encourage people to set and, strive to
reach, goals and ambitions. For some this will mean managing domestic chores
while for others it will mean gaining more access to day care services, an
opportunity to learn new skills such as computing, sewing, gardening or a
musical instrument.
At SweetTree, Care Managers sit
at the core of the multidisciplinary services we provide. All of
SweetTrees Clients are allocated a dedicated Care Manager who oversees
the care given and takes overall responsibility for managing the
multidisciplinary input provided, both from members of the SweetTree team and
external agencies.
SweetTree Home Care Services
provides support to individuals with:
- Mild, moderate and severe disabilities
- More complex needs such as:
Autistic
Spectrum Disorder Down Syndrome Challenging Behaviour
Severe Hearing Impairment Compounding Mental Health Needs
- Live-in or Live-out care needs
- Short or long term care requirements
