At SweetTree Home Care Services we understand that many people’s lives are affected by disabilities or illnesses that have an impact on their ability to live what others might consider to be a ‘normal life’ For those with physical and neurological disabilities it is often a split second event or unexpected illness that redefines an otherwise bright and exciting future.

Although, we know that nothing is ever the same after a life changing event or diagnosis like this, we also believe that the possibilities for a positive and fulfilling future remain. We pride ourselves on seeing opportunities rather than obstacles and reflecting this, our prime objective is to assist our clients establish and meet the goals that they set for themselves and their future; regardless of their level of need or dependency.

At SweetTree, we want our clients to continually improve, but also to have fun. The care and support that we provide at SweetTree is based on Person Centred Planning. Helping clients to achieve the goals they set for themselves is paramount, regardless of whether this means focusing on education, receiving vocational training, participating in social activities of daily living such as a day out shopping, a trip out to meet relatives or friends for a meal, or something less active like listening to music or being read to by a carer.

At SweetTree however, we view a move from a residential setting back into the community not as the end point but rather as the starting point; from which new opportunities and a fuller and more active life can be pursued. The key to success in helping someone back into the community is careful planning and a multi-disciplinary approach being adopted, and this is exactly what SweetTree provides.

Regardless of how much pre-planning work is done, leaving the supportive environment of in-patient rehabilitation can be a daunting prospect for many. To facilitate a client’s move back into the and to help those living in the community to operate more independently, SweetTree operates a number of supported living units, adapted to reflect an independent living setting.

SweetTree’s team of Care Managers sit at the core of the multidisciplinary services provided. All of SweetTree’s clients are allocated a dedicated Care Manager who oversees the care given and takes overall responsibility for managing the multidisciplinary input provided from both members of the SweetTree team and external agencies. Our care team’s primary objective is to assist those we look after to remain in their own home for as long as they wish with dignity, independence and control over their life.
